Operational sequence

  1. Explain what you want to do and why in plain language.
  2. Offer a real choice, including delay, refusal, or an alternative when feasible.
  3. Check permission before changing distance, lighting, sound, or participants.
  4. Watch for withdrawal of consent and stop or renegotiate.
  5. Record any unavoidable limit and the least restrictive reason for it.

Failure states

  • Treating silence as consent
  • Offering a false choice
  • Using clinical authority as automatic permission
