More visible failures can mean more oversight

A system with public inquiries, litigation, inspector reports, and declassified records may accumulate many documented failures. Those records support specific criticism and may also demonstrate correction capacity. A system with few public failures may be effective, opaque, censored, poorly documented, or all of these; the evidence alone cannot decide.

Track distance from the event and institution

Official statements, court findings, multilateral records, local journalism, scholarship, advocacy reports, technical telemetry, foreign-government attribution, defectors, exile groups, anonymous sources, and social media have different access and incentives. The site records source type, independence, language, date, and corroboration rather than flattening them into a citation count.
