Operational sequence

  1. Collect multi-year records and the person’s own chronology.
  2. Mark mood episodes and psychosis outside them.
  3. Record competence, work, relationships, and recovery as carefully as impairment.
  4. Verify harassment or surveillance on its own evidence track.
  5. Leave the formulation provisional when the timeline is incomplete.

Failure states

  • Diagnosing from one dramatic statement
  • Treating negative symptoms as laziness
  • Assuming organized work rules out psychosis
  • Archiving errors without revision
