Operational sequence

  1. Record the person’s baseline and ordinary functioning.
  2. Normalize every event to UTC and mark the source.
  3. Track sleep, mood, substances, medications, illness, injury, trauma reminders, and operational incidents on separate lanes.
  4. Distinguish observation, interpretation, and later chart language.
  5. Revise the working formulation when the sequence changes.

Failure states

  • Choosing a diagnosis from content alone
  • Treating the first note as final
  • Ignoring late-onset or fluctuating medical signs
  • Deleting contradictory versions
